New Patient Registration

If you live in our catchment/practice area ( please see our map below) we will accept you on our practice list.

It is practice policy to accept whole families only, who reside at the same address. This enables us to care for individuals within the family and have contact with the family as a whole should the need arise.

If you wish to register at the practice, you should bring your medical card to the surgery. If your do not have a medical card, a form will be provided or you can download the GMS1 form below.  To avoid delay in registering with the surgery please complete all relevant boxes and ensure that the form has been signed before posting or bringing in to the surgery.

You will then be registered with an individual doctor and will receive notification of your named GP.

Practice Area

The geographical boundry of our practice area in which patients will be entitled to register is:

Crow Wood Park, Rochdale Road to King Cross, Haugh Shaw Road, Savile Park Road, Oxford Road, Hunger Hill, Water Lane, Trooper Lane to Bank Top, Pinnar Lane and Towngate Southowram, West Lane, Park Lane and Exley Bank to Wakefield Road including Copley to Upper Bolton Brow and Crow Wood Park.

We suggest that patients register with the surgery most appropriate to their address as detailed on the map. Both surgeries have disabled access.
